Quiz Privacy Notice – Janna Block Wellness LLC

1. Information We Collect

When you complete the Quiz, we may collect:

• first name
• email address
• last name and phone number, only if you provide them through another interaction with us (the current Quiz form asks only for your first name and email)
• marketing attribution parameters (such as UTM values and click identifiers) if present in the link you used to reach the Quiz
• your quiz responses
• your quiz result category, including top pattern and stage
• related summary information, including symptom count, age range, and cycle status
• records of legal acceptance and follow-up communication authorization
• consent metadata, including timestamps, consent version, user agent, source URL, and server-captured IP address if available

2. How We Use Your Information

We maintain two distinct uses for the information you submit.

Operational Use. We use your information to score the Quiz, generate your results, create and store your lead record, respond to your request, operate and improve the Quiz, maintain business and compliance records, and support the systems used to run the Quiz.

Authorized Follow-Up Communications Use. If you voluntarily check the optional authorization on the Quiz, we may use the information you submit through the Quiz to email you Janna's free 3-Day Calm Plate Guide and occasional educational support about midlife hormone health. If you separately provide a phone number and authorize it through another interaction with us, we may also contact you by phone call or text message; the current Quiz form does not collect a phone number for this purpose.

You are not required to authorize follow-up communications in order to receive your Quiz results.

3. Data Storage and Transmission

Your information is first submitted through the app and stored in the systems used to operate the Quiz, including Base44. If — and only if — you grant the optional follow-up communication authorization, your contact information and limited quiz-routing information (such as your result category) may then be transmitted to GoHighLevel for lead management and authorized follow-up communications. If you do not grant that authorization, your contact and quiz information is not transmitted to GoHighLevel.

4. Sensitive Personal Data

Some information collected through this Quiz may be considered sensitive personal data because it relates to symptoms, hormone concerns, and other health-related information. We process this information only for the purposes described in this Notice and based on the consent choices you make in the app.

5. Consent Records

We store records of your legal acceptance and follow-up communication authorization for recordkeeping, audit, compliance, and internal business purposes.

6. No Sale of Data

We do not sell your Quiz information.

7. Service Providers

We may use trusted service providers and platforms to host the Quiz, store submissions, manage communications, support business operations, and maintain security and compliance. These may include Base44 and GoHighLevel, along with other service providers reasonably necessary to operate our business.

8. Your Choices and Requests

You may contact us to:

• ask questions about this Notice
• request correction of your contact information
• unsubscribe from emails
• revoke follow-up communication authorization going forward
• request deletion of Quiz information, subject to legal, compliance, and operational limits

9. Security

We use reasonable administrative, technical, and physical safeguards designed to protect the information collected through this Quiz. No online or electronic storage system can be guaranteed to be completely secure.

10. Retention

We retain your information for as long as reasonably necessary to fulfill the purposes described in this Notice, unless a longer period is required by law or a shorter period is required by a valid deletion request or internal policy.

11. Children

The Quiz is not directed to individuals under 18 years of age.

12. Changes to This Notice

We may update this Privacy Notice from time to time. Any updated version will be posted in the app with a revised Last Updated date.
