These Quiz Terms of Use govern your access to and use of the hormone symptom quiz and related pages, features, and content provided by Janna Block Wellness LLC ("Company," "we," "us," or "our") in connection with the services of Janna Block, APRN, FMACP. By using the Quiz or submitting information through it, you agree to these Terms.
The Quiz is an educational and informational tool only. It is intended to help users better understand possible hormone-related patterns based on self-reported information. The Quiz is not medical advice, diagnosis, or treatment, and it is not intended to diagnose, treat, cure, or prevent any disease or medical condition.
Completing the Quiz, viewing results, downloading materials, or submitting your information does not create a provider-patient relationship with Janna Block Wellness LLC, Janna Block, APRN, FMACP, or any affiliated person or entity.
Quiz results are generated algorithmically from your self-reported answers. They do not take into account your full medical history, medications, laboratory results, physical examination, or other clinical findings. You should not rely on Quiz results as a substitute for evaluation, diagnosis, or treatment by a licensed healthcare professional.
You agree to provide information that is truthful, accurate, and complete to the best of your knowledge. Inaccurate or incomplete information may affect the usefulness, accuracy, or relevance of your Quiz results.
You must be at least 18 years old to use the Quiz.
By submitting the Quiz, your responses and contact information are transmitted through the app and stored in the systems used to operate the Quiz, including Base44. Transmission to GoHighLevel occurs only if you grant the optional follow-up communication authorization: in that case, your contact information and limited quiz-routing information (such as your result category) may be sent to GoHighLevel for lead management and authorized follow-up communications. If you do not grant that authorization, your contact and quiz information is not sent to GoHighLevel. Some information you provide may be considered sensitive personal data because it relates to symptoms, hormone concerns, or other health-related information.
If you check the optional authorization on the Quiz, Janna Block Wellness LLC may use the information you submit through the Quiz to email you Janna's free 3-Day Calm Plate Guide and occasional educational support about midlife hormone health, including follow-up support and invitations to speak with Janna. The current Quiz form collects your first name and email and does not collect a phone number; if you separately provide a phone number and authorize it, you may also receive communications by phone call or text message. If you do not provide the optional authorization, your Quiz-result information will not be used for those follow-up communications except as otherwise permitted by law.
We store records of your consent choices, including the version of the legal terms presented to you at the time of submission, for operational, recordkeeping, audit, and compliance purposes.
We do not guarantee that the Quiz results will be accurate for your specific circumstances or that any particular health, symptom, scheduling, or service-related outcome will result from using the Quiz.
The Quiz is not intended for urgent or emergency concerns. If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.
All Quiz content, wording, design, layout, graphics, scoring logic, algorithms, outputs, branding, and educational materials are owned by or licensed to Janna Block Wellness LLC unless otherwise stated. You may not copy, reproduce, distribute, republish, modify, or commercially exploit any part of the Quiz without prior written permission.
The Quiz is provided on an "as is" and "as available" basis. To the fullest extent permitted by law, Janna Block Wellness LLC disclaims all warranties, express or implied, regarding the Quiz and its results, including any implied warranties of merchantability, fitness for a particular purpose, accuracy, or non-infringement.
To the fullest extent permitted by law, Janna Block Wellness LLC and its owners, providers, employees, contractors, affiliates, and agents shall not be liable for any indirect, incidental, special, consequential, punitive, or exemplary damages arising out of or related to your use of the Quiz, your reliance on the results, or your inability to use the Quiz.
These Terms are governed by the laws of the State of Minnesota, without regard to conflict of law principles.
We may update these Terms from time to time. Any updated version will be posted in the app with a revised Last Updated date. Your continued use of the Quiz after an update means you accept the revised Terms.
